We have an open position for a Licensed Social Worker (LSW) or Registered Nurse (RN) in MN to oversee PCAs and manage care plans. The candidate we are looking for is someone who is independent, compassionate, loves what he/she do knowing that they make differences in people’s lives. And most of all, someone who wants to grow with us as we are a new small agency but we are planning to only grow more from here.

The schedule is flexible as you set your own hours/time. This position is a contract paid per diem.

 

Job Description:

Role and Duties:

The QP performs the duties of training, supervision, and evaluation of the personal care assistance staff and evaluation of the effectiveness of personal care assistance services.

  • Develop care plan

  • Train, orient and evaluate new PCAs

  • Train PCAs on new procedures, equipment, and based on changes to the care plan

  • Oversee the delivery of PCA services

  • Ensure health and safety needs are met

  • Document changes in condition and request reassessments

  • Supervise 16- and 17-year old PCAs

  • Conduct supervisory home visits with PCAs

 

The QP is responsible for overseeing the delivery of and evaluating the effectiveness of PCA services including:

  • The satisfaction level of the recipient/responsible party

  • The month-to-month plan for use of PCA services

  • The care plan

  • The documentation of PCA services provided

 

Desired Skills & Experience:

Requirement:

  • Education: BA in Social Work/ Registered Nurse

  • Licensure: Minnesota's licensed

  • 1 Year of experience in Home Care or related field 

  • Certification: Complete the Step for Success Certification

  • Pass background check

  • Valid Driver’s License

**Bonus is to be split into 2 payments. First $500 is to be paid after training completion. The remaining $500 is to be paid after 1 year of employment.
